Make a Claim
Making a claim couldn’t be simpler, just select the form you need below and complete it online – this will be sent directly to your broker. You can also download the form and return it to claims@northco.co.nz or print a copy and send it to us via post.
General Claim Form
If you would prefer to complete a hardcopy of the form you can download the general claims form and send it back to us in the post, via fax or by scanning and emailing it.
We will be in touch to acknowledge that we have received your claim.
Motor Vehicle Claim Form

If you need to make a life or health insurance claim we ask that you get in touch with us directly by
